DTS application is composed of five Parts;
  Part One- Personal Details
  Part Two- Personal History Questions
  Part Three- References Forms
  Part Four- Payment Schedule and Refund Policy
  Part Five- Personal Medical History and Doctor’s Evaluation

There are two ways of applying:

1. You can download the DTS Full Application Part 1-5 fill it out and email, fax or mail it in or;

2. You can use our Online Application and then download the other associated forms to email, fax or mail in at a later date.

By using the Online Application you are able to complete Part One & Two and it enables the registrar to start the application process immediately.

In order to complete the rest of the application, you will then need to download the Medical History, Doctor's Evaluation and Reference Forms and Course Information Handbook.

We will also then need a:
1.Copy of your passport
2.Police Criminal Record Check in English.
3.English Proficiency Test (If English is not first language)
4.$25.00 AUD (35.00 AUD married couple) Application Fee. This is a non-refundable fee which covrs associated administration expenses.
